PLEASE SUBMIT THIS FORM BY AUGUST 27, 2025. PLEASE INCLUDE FIRST AND LAST NAME FOR EACH GUEST SO WE CAN PRINT A BADGE FOR EACH PERSON ATTENDING.

Please make sure to only capitalize the first letter in the names fields. All fields with an asterisk MUST BE FILLED OUT in order for your form to be submitted. PLEASE DO NOT REGISTER MORE THAN ONCE. Children below 3rd grade are not issued badges for the adult services. We encourage you and your younger family members to experience the young children's family service for non-readers or the youth experience for gardes 3-6th. Infant seats and strollers are not permitted in the auditorium except during the morning children's service. Registration is required for both the children's services and the youth experiences.
